A GP Management Plan is a document your doctor prepares with you for a condition that has lasted, or is likely to last, at least six months. It sets out your health problems, the goals you have agreed, the treatments and services required, and what you will do yourself.

Who is eligible

Anyone with a chronic or terminal medical condition. In practice that means diabetes, heart disease, high blood pressure, asthma, COPD, arthritis, chronic kidney disease, osteoporosis, stroke, cancer, chronic pain and many mental health conditions. Your GP will confirm whether your situation qualifies.

Good to know

The most valuable part for most people is the allied health access: up to five Medicare-rebated visits per calendar year to physiotherapy, dietetics, podiatry, exercise physiology and more.

Team Care Arrangements

Where two or more other providers are involved in your care, a Team Care Arrangement is added. This is what formally brings in the allied health rebates and makes sure everyone is working from the same plan rather than in parallel.

Allied health under your plan

Our physiotherapists Smit Patel and Rutvik Lathiya, podiatrist Baqer Al-Temeemi, dietitian Anagha Chaudhari and counsellor Francesca Palazzolo all work from this clinic, which means your allied health team can read the plan, talk to your GP and refer back easily. Bring your referral letter and care plan to each allied health appointment so the Medicare rebate can be claimed.

Reviews

Plans are reviewed every three to six months. The review checks progress, adjusts goals and renews the allied health referrals. A plan that is never reviewed stops being useful fairly quickly.
