Looking after the whole picture.

As we get older, several conditions and medicines often overlap. Regular reviews with one GP keep everything working together.

Dr Mohammed Hossain, with more than thirty years in general practice, focuses on chronic and complex conditions, and all of our GPs look after older patients and their families. Longer appointments are available so nothing is rushed, and family members or carers are welcome to attend.

75 and over health assessment

A comprehensive, bulk billed check for patients aged 75 and over, usually done partly with our practice nurse. It covers physical health, medicines, memory and mood, mobility and falls risk, hearing and vision, continence, nutrition, social support and safety at home, and produces a plan you and your GP agree on.

Medication reviews

Many older adults take five or more medicines. Your GP reviews each one for benefit, dose, interactions and side effects, and can arrange a Home Medicines Review with a pharmacist. Bring all your medicines or a current list to every appointment.

Good to know

A fall is often the first sign that something else needs attention. Tell your GP about any trips, stumbles or dizziness, even if you were not hurt.

Staying independent

Falls prevention with our physiotherapists, bone health and osteoporosis treatment, hearing and vision checks, continence support, nutrition with our dietitian, driving assessments, advance care planning and help navigating My Aged Care and home support services.
