A chronic condition management plan turns good intentions into a schedule: what to check, when, and who is helping.
Nearly half of Australians live with at least one chronic condition. The difference between a condition that is controlled and one that causes complications is usually consistent, planned care. Dr Mohammed Hossain has more than thirty years in general practice and focuses on complex and chronic conditions including heart, gut and kidney disease.
If you have a condition expected to last six months or more, your GP can prepare a management plan. It records your conditions, medicines, goals and the checks you need, and coordinates care with other providers. Eligible patients then receive Medicare rebates for up to five allied health visits each year, including our on-site physiotherapists and dietitian.
An annual cycle of care covering blood glucose (HbA1c), blood pressure, cholesterol, kidney function, weight, eye and foot checks, and medication review. Our dietitian Ms Anagha Chaudhari supports meal planning and weight, and your GP coordinates referrals for eye checks, podiatry and diabetes education. Type 2 diabetes prevention is also part of what we do.
Bring your blood pressure or glucose readings from home. Trends over time tell your GP more than a single number in the clinic.
Blood pressure and cholesterol management, heart failure and atrial fibrillation monitoring with ECG on site, asthma and COPD with spirometry and action plans, chronic kidney disease monitoring, arthritis and chronic pain with physiotherapy support, and thyroid and other long-term conditions. We also arrange Home Medicines Reviews and coordinate with your specialists.
