Bulk billing explained: what Medicare covers at a GP visit in 2026

If you have searched for a "bulk billing doctor near me" in Toongabbie, Pendle Hill or Wentworthville, you have probably seen the phrase on a dozen clinic signs. It is one of the most searched terms in Australian general practice, and one of the least well explained. This guide sets out how bulk billing works, who is eligible and what to expect when you book at a bulk billing medical centre.

What bulk billing means

Medicare sets a schedule fee for every GP service, from a standard consultation to a care plan or a skin procedure. When a clinic bulk bills, the doctor accepts the Medicare benefit as full payment for that service. The claim is sent to Medicare on your behalf, Medicare pays the practice directly, and you leave without paying anything. There is no gap fee, no upfront payment and no rebate to chase.

At a mixed or private billing practice the doctor charges more than the Medicare benefit. You pay the full fee, Medicare refunds part of it, and the difference is the out-of-pocket "gap". Bulk billing removes that gap entirely.

Who is eligible

Bulk billing is available to anyone with a current Medicare card. That includes Australian citizens, permanent residents and visitors from countries with a Reciprocal Health Care Agreement who have enrolled in Medicare. Children are covered under a parent's card until they have their own. Please bring your card, or have your Medicare number to hand, so reception can process the claim.

If you do not have a valid Medicare card, for example because you are on a visitor or student visa without reciprocal cover, a private fee applies and you may be able to claim from your overseas health insurer. Reception will explain any fee before your appointment.

Bulk billing, mixed billing and private billing compared

Practices describe themselves in three ways. A bulk billing practice accepts the Medicare benefit for all Medicare-listed services, so there is never a gap for card holders. A mixed billing practice bulk bills some patients, often children, pensioners and concession card holders, and charges a gap for others. A private billing practice charges a gap for everyone. When you see "bulk billing available" on a sign, it usually means mixed billing, so it is worth asking reception exactly who is bulk billed before you book.

What the Medicare rebate covers behind the scenes

Every Medicare item has a schedule fee and a benefit, which is the amount Medicare pays. For general practice the benefit is 100 percent of the schedule fee, and since late 2023 the government has tripled the bulk billing incentive paid to practices for children under 16, pensioners and concession card holders.
